Medical Assistant

Responsibilities

• Facilitate in-room telemedicine visits and schedule provider appointments
• Prepare and support residents during provider visits, including positioning and documentation
• Update EHRs with medical histories to support care plans and visit encounters
• Support transitions of care, including referrals and follow-up appointments
• Facilitate prior authorizations and assist residents with ACO Voluntary Alignment forms
• Reinforce provider instructions and educate residents on nutrition, fall prevention, and medication reminders
• Maintain accurate documentation in the EMR and support regulatory compliance
• Coordinate with facility Case Management and Social Work teams on discharge planning
• Develop relationships with SNF administrators and hospital departments

Requirements

• Graduate of an accredited Medical Assistant (MA) program
• Experience charting in an EMR
• Knowledge of geriatric medical practice and terminology
• Advanced computer skills including Excel and Google Workspace
• Ability to lift up to 20 lbs. and assist with resident transfers
• Ability to travel to patient locations (home, hospital, SNF)

Preferred Qualifications

• Certified Medical Assistant (CMA)
• Prior experience in a clinical or care-coordination setting
• Experience as a Health Plan or Hospice Liaison
• Managed Care experience
