Responsibilities

• Empower patients to reach health goals using motivational interviewing and continuous glucose monitor (CGM) data
• Collaborate with the Ryse Health team on care planning across diet, exercise, sleep, stress management, and medication
• Communicate with patients through video calls, in-person meetings, telephone, and messaging
• Collect critical patient data including vital signs, A1c, and retina scans
• Educate patients on using CGMs and medications and how to treat hypoglycemia
• Support patients to better cardiometabolic health in a team-based care context

Requirements

• 1+ years of experience in a clinical setting
• Ability to balance multiple competing priorities and attention to detail
• Self-motivated and adaptable to changing processes
• Full COVID-19 and flu vaccination is required

Preferred Qualifications

• Medical Assistant certification
• Motivational interviewing training
• Phlebotomy experience
