About the Company
Ennoble Care is a mobile primary care, palliative care, and hospice service provider. Their clinicians provide a continuum of care for patients with chronic conditions and limited mobility, offering programs like remote patient monitoring and behavioral health management.
Responsibilities
- Coordinate patient-centered care activities using the EMR system
- Support providers through accurate documentation and follow-up tracking
- Serve as a communication link between providers and internal care teams
- Communicate patient needs and urgent concerns to the clinical team
- Assist with care transitions, referrals, and external partner coordination
- Maintain accurate documentation in accordance with established workflows
Requirements
- Active clinical certification or licensure (PCT, CNA, MA, CMA, LPN, or RN)
- Minimum 1 year of experience in clinical/administrative or care coordination roles
- Proficiency with Microsoft 365 (Outlook, Word, Excel)
- Strong knowledge of healthcare terminology
- Ability to work onsite in Voorhees, NJ, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST
- Ability to pass a background check and drug screen
