About the role

Responsibilities

  • Develop and grow Medicaid referral pipelines across NYC boroughs, Nassau, and Westchester
  • Leverage existing relationships within home care, skilled nursing, and long-term care networks
  • Build and maintain strong partnerships with nursing home social workers, Medicaid coordinators, admissions teams, and discharge planners
  • Focus outreach efforts on hospital-based support groups to generate high-quality patient referrals
  • Educate referral sources on Medicaid eligibility services and value-added consulting support
  • Maintain consistent field presence across hospitals, SNFs, and community healthcare organizations
  • Identify and cultivate new referral opportunities within assigned territory
  • Track outreach activity, referral performance, and pipeline growth
  • Collaborate with internal teams to ensure smooth client onboarding and case progression
  • Represent the organization as a trusted Medicaid navigation partner within the healthcare community

Requirements

  • 3–10+ years of experience in healthcare marketing, preferably within home care or skilled nursing environments
  • Strong existing network within NYC healthcare systems (hospitals, SNFs, home care agencies)
  • Demonstrated experience generating referrals in Medicaid, long-term care, or related healthcare services
  • Established book of business strongly preferred
  • Deep understanding of healthcare referral processes, including discharge planning and Medicaid coordination workflows
  • Strong relationship-building and communication skills with clinical and administrative stakeholders
  • Ability to work independently in a field-based, territory-driven role
  • Must be located in or near Queens, Brooklyn, Manhattan, Nassau, or Westchester
  • Background in home care marketing is highly preferred over hospital-only outreach experience
