MDS Coordinator

Responsibilities

• Review clinical documentation and hospital records to ensure completeness and compliance with CMS regulations
• Review and validate Patient-Driven Payment Model (PDPM) information for accuracy and reimbursement optimization
• Evaluate the quality of care being provided to long-term care residents through detailed assessment and documentation
• Complete MDS assessments and ensure their accuracy and timely submission in compliance with state and federal regulations
• Maintain up-to-date knowledge of Medicare and Medicaid criteria and serve as a resource for nursing staff
• Communicate regulatory changes and provide guidance and training as needed
• Monitor Medicare assessment schedules and nursing documentation to ensure consistency and compliance

Requirements

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a current state license
• Minimum of 2 years of experience in long-term care
• Strong knowledge of MDS 3.0, PDPM, and CMS guidelines
• Proficient in electronic medical records and MDS software
• Excellent analytical, organizational, and communication skills
