Master Social Worker - MSW

Responsibilities

• Provide psychosocial services, patient assessment, and counseling to dialysis patients to promote rehabilitation and improved quality of life
• Collaborate with the interdisciplinary healthcare team to support patients and families facing the emotional and psychological impacts of Chronic Kidney Disease
• Develop and implement care plans to address barriers to treatment and meet patient rehabilitation goals
• Provide supportive counseling and educational services, including guidance for patients seeking kidney transplants
• Facilitate referrals to community resources such as home health services and vocational rehabilitation
• Assist patients with understanding advance directives and participate in discussions regarding DNR status
• Monitor and report on quality indicators related to patient adherence, quality of life trends, and service recovery
• Coordinate with insurance coordinators to educate patients on payment options and resolve coverage issues

Requirements

• Master's degree in Social Work (MSW)
• Must hold a state-required social work license
• 2–5 years of related professional experience
