Master Social Worker - MSW

Responsibilities

• Provide psychosocial services and counseling to in-center and home dialysis patients to promote positive adjustment and improved quality of life
• Assess patients' psychosocial status, strengths, and areas of need as part of a comprehensive interdisciplinary care plan
• Deliver supportive counseling and educational interventions regarding kidney disease, transplant processes, and End Stage Renal Disease (ESRD)
• Facilitate referrals to community resources, including vocational rehabilitation, home health services, and financial assistance programs
• Collaborate with the interdisciplinary healthcare team to address non-adherence and monitor quality of life trends
• Assist patients with advance directives and participate in discussions regarding DNR status in collaboration with physicians and nurses
• Document all social work interactions, interventions, and patient assessments in accordance with company policy

Requirements

• Master's degree in Social Work (MSW)
• Valid state-required social work license
• 2-5 years of related professional experience
