Master Social Worker - MSW

Responsibilities

• Provide psychosocial services and supportive counseling to in-center and home dialysis patients.
• Conduct patient assessments to identify psychosocial strengths, needs, and barriers to treatment.
• Collaborate with the interdisciplinary healthcare team to develop and implement care plans that improve quality of life.
• Educate patients and families on kidney disease, treatment modalities, and transplant processes.
• Facilitate referrals to community resources, including vocational rehabilitation and home health services.
• Assist patients with advance directives and participate in discussions regarding DNR status.
• Monitor and report on quality indicators such as treatment adherence and patient satisfaction.
• Coordinate with insurance coordinators to address patient coverage and financial assistance needs.

Requirements

• Master’s degree in Social Work (MSW).
• Valid state-required social work license.
• Knowledge of social work theory and human behavior.
• Ability to meet the applicable scope of practice and licensure requirements for the state of employment.

Preferred Qualifications

• 0–2 years of related experience in a clinical or healthcare setting.
• Experience working with patients facing chronic illness or End Stage Renal Disease (ESRD).
