Master Social Worker - MSW

Responsibilities

• Provide psychosocial services and counseling to in-center and home dialysis patients to promote positive adjustment and improved quality of life.
• Conduct patient assessments to identify psychosocial strengths, needs, and barriers to treatment.
• Collaborate with the interdisciplinary healthcare team to develop and implement effective care plans.
• Provide educational support regarding kidney disease, treatment modalities, and the transplant referral process.
• Assist patients in accessing community resources, including vocational rehabilitation and home health services.
• Facilitate discussions regarding advance directives and DNR status in collaboration with physicians and families.
• Monitor and report on quality indicators such as treatment adherence and patient satisfaction.
• Act as a liaison between patients and insurance coordinators to address coverage and financial assistance needs.

Requirements

• Master's degree in Social Work (MSW).
• Valid state-required social work license.
• 2–5 years of related professional experience.
