Master Social Worker - MSW

Responsibilities

• Provide psychosocial services and supportive counseling to dialysis patients to improve quality of life and treatment adherence.
• Conduct patient assessments to identify psychosocial strengths, needs, and barriers to optimal treatment outcomes.
• Collaborate with the interdisciplinary healthcare team to develop and implement effective care plans.
• Educate patients and families on kidney disease, treatment modalities, and psychosocial issues related to End Stage Renal Disease (ESRD).
• Facilitate referrals to community resources, including vocational rehabilitation, home health services, and transplant programs.
• Assist patients with navigating insurance, financial assistance, and advance directive discussions.
• Monitor and report on quality indicators such as treatment adherence and patient satisfaction.
• Document all social work interactions and interventions in accordance with company policy.

Requirements

• Master's degree in Social Work (MSW).
• Must hold a valid state-required social work license.
• Must meet all applicable state board and licensure requirements for the scope of practice.
• 0–2 years of related experience (entry-level).
