Master Social Worker - MSW

Responsibilities

• Provide psychosocial services and supportive counseling to dialysis patients and their families to improve quality of life.
• Conduct patient assessments to identify psychosocial strengths, needs, and barriers to treatment.
• Collaborate with the interdisciplinary healthcare team to develop and implement effective care plans.
• Educate patients and families on kidney disease, treatment modalities, and transplant processes.
• Facilitate referrals to community resources, including vocational rehabilitation and home health services.
• Assist patients with advance directives and participate in discussions regarding DNR status.
• Support patient adherence to treatment through psychosocial interventions and quality monitoring.
• Coordinate with insurance coordinators to address patient coverage and financial assistance needs.
• Maintain accurate documentation of all social work interactions and interventions.

Requirements

• Master's degree in Social Work (MSW).
• Must hold a valid state-required social work license.
• Must meet all state scope of practice board and licensure requirements.
• 0–2 years of related experience (entry-level).
