Master Social Worker - MSW

Responsibilities

• Provide psychosocial services, counseling, and care planning for dialysis patients
• Assess patient needs, strengths, barriers, and adjustment to treatment
• Collaborate with the interdisciplinary care team to support quality outcomes and patient satisfaction
• Educate patients and families on ESRD, treatment options, rights, responsibilities, and available resources
• Assist with referrals to community, vocational, insurance, and financial support services
• Document assessments, interventions, and care plans in accordance with policy
• Participate in quality improvement activities, team huddles, and monthly quality reviews

Requirements

• Master’s degree in Social Work
• State-required social work license
• Knowledge of social work theory and accepted social work practice methods
• 0–2 years of related experience
• Ability to work onsite and travel as needed
• Strong communication, documentation, and collaboration skills

Preferred Qualifications

• Experience supporting patients with chronic illness or dialysis care
• Familiarity with transplant referral processes and advance directives
• Knowledge of community resources, vocational rehabilitation, and insurance assistance programs
