Master Social Worker - MSW

Responsibilities

• Provide psychosocial services to dialysis patients and support adjustment to chronic kidney disease
• Assess patient needs, strengths, barriers, and treatment adherence concerns
• Participate in care planning with the interdisciplinary team to improve outcomes and quality of life
• Offer supportive counseling, education, and referrals to community and vocational resources
• Support transplant education, advance directives, and continuity of care
• Document assessments, interventions, and patient/family interactions accurately
• Participate in quality improvement activities, team huddles, and monthly quality reviews

Requirements

• Master’s degree in Social Work
• Current state-required social work license
• Knowledge of social work theory and accepted clinical methods
• 0–2 years of related experience
• Ability to work onsite in a clinical dialysis setting

Preferred Qualifications

• Experience working with patients with chronic illness or ESRD
• Familiarity with dialysis care, transplant referral processes, and community resources
• Strong communication, counseling, and interdisciplinary collaboration skills
