Master Social Worker - MSW

Responsibilities

• Provide psychosocial services, counseling, and care planning for dialysis patients
• Assess patient needs, strengths, barriers, and adjustment to chronic kidney disease
• Collaborate with the interdisciplinary care team to support treatment adherence and quality of life
• Educate patients and families on ESRD, treatment options, advance directives, and available support services
• Connect patients with community, vocational, insurance, and financial resources
• Document assessments, interventions, and care plans in accordance with policy
• Participate in quality improvement activities and monthly quality review meetings

Requirements

• Master’s degree in Social Work
• Current state-required social work license
• Knowledge of social work theory and accepted social work practice methods
• Ability to provide supportive counseling within scope of training and licensure
• 0–2 years of related experience

Preferred Qualifications

• Experience working with dialysis, chronic illness, or renal care patients
• Familiarity with care coordination in an interdisciplinary healthcare setting
• Knowledge of community resources, vocational rehabilitation, and insurance assistance programs
