Master Social Worker - MSW

Responsibilities

• Provide psychosocial services, counseling, and care planning support for dialysis patients and their families
• Assess patient psychosocial needs, strengths, barriers, and adjustment to chronic kidney disease and treatment
• Collaborate with the interdisciplinary care team to improve treatment outcomes, quality of life, and patient satisfaction
• Educate patients, families, and staff on psychosocial issues, treatment modalities, rights, responsibilities, and available resources
• Support transplant referrals, advance care planning, and referrals to community, vocational, and financial resources
• Document assessments, interventions, and care coordination in accordance with social work and facility policies
• Participate in quality improvement activities, monthly reviews, and team huddles

Requirements

• Master’s degree in Social Work
• Current state-required social work license
• Knowledge of social work theory and accepted methods of practice
• 0–2 years of related experience
• Ability to work onsite and travel as needed

Preferred Qualifications

• Experience supporting patients with chronic illness or dialysis care
• Familiarity with ESRD, transplant coordination, and community resource referral processes
• Strong communication, counseling, and interdisciplinary collaboration skills
