Master Social Worker - MSW

Responsibilities

• Provide psychosocial services, counseling, and care planning for dialysis patients
• Assess patient needs, strengths, barriers, and adjustment to chronic kidney disease and treatment
• Collaborate with the interdisciplinary care team to support rehabilitation, adherence, and quality of life
• Educate patients, families, and staff on psychosocial issues, treatment options, and available resources
• Assist with referrals to community, vocational, insurance, and financial support services
• Document assessments, interventions, and patient progress in accordance with policy
• Participate in quality improvement activities, team huddles, and monthly quality reviews

Requirements

• Master’s degree in Social Work
• Current state-required social work license
• Knowledge of social work theory and accepted methods of practice
• 0–2 years of related experience
• Ability to work onsite in a clinical dialysis setting

Preferred Qualifications

• Experience supporting patients with chronic illness or ESRD
• Familiarity with dialysis care, transplant referral processes, and community resources
• Strong communication, counseling, and interdisciplinary collaboration skills
