Master Social Worker - MSW

Responsibilities

• Provide psychosocial services and supportive counseling to in-center and home dialysis patients
• Assess patients' psychosocial status, strengths, and needs to assist in care planning and rehabilitation
• Collaborate with an interdisciplinary healthcare team to promote patient quality of life and treatment adherence
• Educate patients and families on the emotional and behavioral impacts of Chronic Kidney Disease
• Facilitate referrals to community resources, including vocational rehabilitation and home health services
• Assist patients with the transplant referral process and manage transplant waitlist coordination
• Support patient understanding of advance directives and participate in DNR status discussions
• Document all social work interactions, interventions, and patient assessments in accordance with company policy
• Monitor quality indicators such as treatment adherence and service recovery to improve patient outcomes

Requirements

• Master's degree in Social Work (MSW)
• Valid state-required social work license
• Knowledge of social work theory and human behavior
• 0–2 years of related experience (entry-level)
