Master Social Worker - MSW

Responsibilities

• Provide psychosocial services and counseling to in-center and home dialysis patients to promote positive adjustment and rehabilitation.
• Assess patients' psychosocial status, strengths, and needs as part of a comprehensive interdisciplinary care plan.
• Deliver supportive counseling and educational interventions to help patients manage the emotional and behavioral impacts of Chronic Kidney Disease.
• Facilitate the transplant referral process and provide goal-directed counseling for patients seeking transplantation.
• Connect patients and families with community resources, including vocational rehabilitation, home health services, and financial assistance.
• Assist patients with advance directives and participate in discussions regarding DNR status in collaboration with physicians and nurses.
• Monitor quality indicators such as treatment adherence and quality of life trends to support clinical outcomes.
• Collaborate with insurance coordinators to educate patients on payment options and coverage changes.

Requirements

• Master's degree in Social Work (MSW).
• Valid state-required social work license.
• 2–5 years of related professional experience.
