Master Social Worker - MSW

Responsibilities

• Provide psychosocial services and supportive counseling to dialysis patients and their families.
• Conduct patient assessments to identify psychosocial needs, strengths, and barriers to treatment.
• Collaborate with an interdisciplinary healthcare team to develop and implement care plans.
• Educate patients and families on kidney disease, treatment modalities, and transplant processes.
• Facilitate referrals to community resources, including vocational rehabilitation and home health services.
• Assist patients with navigating insurance, financial assistance, and advance directive discussions.
• Monitor and report on quality indicators such as patient adherence and quality of life trends.
• Maintain accurate documentation of all social work interventions and patient interactions.

Requirements

• Master's degree in Social Work (MSW).
• Valid state-required social work license.
• Compliance with all state board licensure and scope of practice requirements.
• 0–2 years of related experience (entry-level candidates welcome).
