Master Social Worker - MSW

Responsibilities

• Provide psychosocial services and supportive counseling to in-center and home dialysis patients.
• Conduct patient assessments to identify psychosocial strengths, needs, and barriers to treatment.
• Collaborate with the interdisciplinary healthcare team to develop care plans that improve quality of life and treatment adherence.
• Educate patients and families on kidney disease, treatment modalities, and transplant processes.
• Facilitate referrals to community resources, including vocational rehabilitation and home health services.
• Assist patients with advance directives and participate in discussions regarding DNR status.
• Support insurance and financial assistance needs by collaborating with Insurance Coordinators.
• Document all patient interactions and interventions in compliance with company policy.

Requirements

• Master’s degree in Social Work (MSW).
• Must hold a valid state-required social work license.
• Compliance with all applicable state licensure and scope of practice board requirements.
• 0–2 years of related experience (entry-level candidates are welcome).
