Master Social Worker

Responsibilities

• Provide psychosocial services and supportive counseling to patients undergoing dialysis treatment
• Assess patients' psychosocial status, strengths, and needs to assist in care planning and rehabilitation
• Collaborate with the interdisciplinary healthcare team to support patient adjustment and quality of life
• Educate patients and families on kidney disease, treatment modalities, and available support services
• Facilitate referrals to community resources, including vocational rehabilitation and home health services
• Assist patients with the transplant referral process and waitlist management
• Support patient adherence to treatment through psychosocial interventions and education
• Document all social work interactions, interventions, and patient assessments in accordance with company policy
• Coordinate with insurance coordinators to address patient coverage and financial assistance needs

Requirements

• Master's degree in Social Work (MSW)
• Valid state-required social work license
• Knowledge of social work theory and human behavior
• 0–2 years of related experience (entry-level)
