Master Level Social Worker

Responsibilities

• Assess patients' psychosocial status, strengths, and needs to support rehabilitation and optimal treatment outcomes
• Develop and implement care plans in collaboration with the interdisciplinary healthcare team
• Provide supportive counseling to patients and families regarding the emotional and psychological impact of Chronic Kidney Disease
• Facilitate patient education on treatment modalities, transplant processes, and kidney disease management
• Connect patients with community resources, including vocational rehabilitation, home health services, and financial assistance
• Assist patients with advance directives and participate in discussions regarding DNR status
• Monitor quality indicators such as treatment adherence and quality of life trends
• Collaborate with insurance coordinators to address patient coverage and payment issues

Requirements

• Master's degree in Social Work (MSW)
• Valid state-required social work license
• 2–5 years of related professional experience
