About the role
Responsibilities
- Support high-risk patients in navigating the healthcare system to reduce ER visits and hospital readmissions
- Perform health condition assessments and initiate medication reconciliation for primary care providers
- Conduct motivational interviewing and self-management goal setting to empower patients
- Facilitate transitions of care from inpatient to outpatient settings, including establishing contingency plans
- Provide patient education and assist with post-discharge needs such as prescriptions, transportation, and DME
- Coordinate with multidisciplinary care teams, including physicians, pharmacists, and social workers, to update plans of care
- Maintain accurate and timely documentation within the electronic medical record
Requirements
- Licensed Practical/Vocational Nurse (LVN/LPN) with an active and unrestricted state license
- Current BLS certification
- 2+ years of clinical or hospital experience (physician's office, clinical, or hospital setting)
- Proven knowledge of chronic diseases, specifically COPD/asthma, diabetes, CHF, and IHD
- Proficient computer skills with experience using electronic medical records
- Strong organizational, prioritization, and interpersonal communication skills
Preferred Qualifications
- IV Certification
- Experience in patient education, motivational interviewing, or self-management goal setting
- Experience with outbound patient telecommunications
- Fluency in both English and Spanish (written and verbal)
