About the role
Responsibilities
- Ensure individual service plans are followed and administer or supervise physician-prescribed treatments
- Perform meaningful rounds to residents and record physician visits on charts
- Administer intramuscular and sub-cutaneous injections, perform in-house testing, and record vital signs
- Document acute episodes or significant changes in resident status
- Handle preparation, administration, and ordering of prescribed medications
- Chart shift activities in accordance with facility policy and state guidelines
- Supervise residents self-administering medication and assist with activities of daily living
- Provide emotional and social support to residents while respecting dignity and confidentiality
- Maintain a safe working environment and adhere to all community policies and procedures
Requirements
- Active LVN/LPN license
- Knowledge of general nursing theory, infection control, and OSHA standards
- Familiarity with medical diagnoses, procedures, and medication side effects
- Ability to lift at least 51 lbs regularly and assist residents with physical needs
- Proficiency in operating computer-related tasks and software applications
- Ability to read, write, speak, and understand English
- Experience with memory care is a plus
