LPN Visit Nurse

Responsibilities

• Provide high-quality clinical services within the scope of practice as defined by state laws and the patient plan of care
• Deliver direct nursing care to acutely and chronically ill patients in their homes
• Document patient visits thoroughly and completely in the electronic medical record per regulatory requirements
• Coordinate patient care activities and report changes in condition to the healthcare team, including physicians and case managers
• Follow all applicable infection control procedures during home visits

Requirements

• Current and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice
• Valid driver's license and vehicle insurance
• Access to a dependable vehicle or reliable public transportation
• Ability to function in diverse home environments regardless of client demographics

Preferred Qualifications

• Previous experience in home health care
• Ability to work flexible hours to meet client needs
• Strong organizational, writing, and communication skills
• Ability to work independently and manage multiple tasks simultaneously
