LPN Transitional Care Coordinator

Responsibilities

• Coordinate care for patients in skilled nursing and rehabilitation facilities using EMR
• Facilitate transitional care management to ensure immediate service initiation upon hospital discharge
• Monitor patients for changes in condition and escalate care as clinically indicated
• Collaborate with providers and facility staff to maintain current care plans
• Maintain a consistent on-site presence to build relationships with residents and families
• Conduct routine and urgent vital sign collection and patient check-ins
• Serve as a liaison between providers, facility staff, residents, and families
• Accurately document patient interactions and perform chart reconciliation
• Support onboarding and training of new Transitional Care Coordinators

Requirements

• Active, unrestricted LPN license in the state of Texas
• Minimum 1 year of patient-facing experience in long-term care, skilled nursing, or transitional care
• Proficiency with Microsoft 365 (Excel, Outlook, Word, Teams)
• Experience with EMR systems; Point Click Care experience preferred
• Valid driver's license and reliable transportation
• Ability to work on-site 5 days per week in the Waco, Texas area
• Ability to pass background check and drug screen
