LPN, Client Care Manager

Responsibilities

• Conduct initial and ongoing comprehensive assessments of clients' needs and living environments
• Develop, document, and implement individualized care plans in collaboration with families and interdisciplinary teams
• Serve as the primary liaison for clients and families to address concerns and inquiries
• Collaborate with the staffing team to assign qualified caregivers based on care plan requirements
• Ensure strict adherence to provincial regulations, licensing standards, and clinical best practices
• Monitor care quality through regular client reviews and in-person follow-up visits
• Coordinate essential services including personal care, medication reminders, and transportation
• Provide on-call phone support to address urgent needs outside of regular business hours

Requirements

• Current and valid Licensed Practical Nurse (LPN) registration/license in Nova Scotia
• 3-5 years of experience in client care management, case management, or a similar healthcare role
• Strong understanding of home care services and provincial regulations
• Exceptional communication, interpersonal, and problem-solving skills
• Proficiency in documentation and maintaining accurate client records
• Ability to work collaboratively within a team environment
