Locum Therapy Assistant

Responsibilities

• Support patient discharges within the Integrated Transfer of Care Team
• Conduct thorough assessments in the community
• Ensure appropriate provision of equipment
• Travel within the community to visit patients
• Work closely with multidisciplinary healthcare professionals to ensure smooth transitions from hospital to home

Requirements

• Experience in a healthcare setting, preferably in community care
• Valid driver’s license and ability to travel within the community
• Strong communication and interpersonal skills
• Relevant qualifications or training as a therapy assistant
• Ability to work independently and as part of a multidisciplinary team
