Life Services Life Coach III

Responsibilities

• Provide one-on-one coaching and development to prepare program participants for employment or education
• Assist members with social determinants of health (SDOH) needs through resource advocacy
• Complete required assessments and maintain accurate documentation in the case management system
• Act as a liaison between participants and social service agencies, government entities, and employers
• Support the development, monitoring, and reporting of individualized Care Plans
• Coordinate services with community leaders and partners to maximize resource utilization
• Conduct regular travel for member, provider, and community-based visits to ensure program effectiveness
• Perform data reporting, analysis, and trend analysis as required

Requirements

• Associate's degree in Social Work, Human Services, Nonprofit Management, or equivalent relevant experience
• 4 years of social service delivery experience
• 3 years of Life Services or highly relatable experience
• Proven experience working with populations living in poverty
• Demonstrated experience with Motivational Interviewing techniques
• Valid driver's license, reliable vehicle, and verifiable insurance
• Proficiency in Microsoft Office Suite (Word, Excel, and PowerPoint)

Preferred Qualifications

• Bachelor's Degree in Social Work, Human Services Administration, or Nonprofit Management
• Professional certification in coaching or related fields (e.g., chemical dependency, workforce development, mental health)
• Certification in working with populations in poverty
