Licensed Vocational Nurse (LVN)

Responsibilities

• Deliver skilled nursing visits in the home within LVN scope under RN/clinical supervision
• Perform focused assessments and monitor for changes in condition
• Escalate clinical changes promptly to the RN/MD/clinical team
• Support medication reconciliation and medication safety education
• Provide patient and caregiver education using teach-back methods
• Support wound care and other skilled tasks per orders and competency
• Coordinate with the interdisciplinary team to ensure timely visits and order compliance
• Document thoroughly and on time in Homecare Homebase (HCHB)

Requirements

• Current unrestricted Texas LVN license
• CPR (BLS) certification
• Current Texas driver’s license with comfort with daily travel
• Strong communication and documentation skills
• Ability to work in a field-based workflow

Preferred Qualifications

• Prior home health or hospice experience
• Familiarity with Homecare Homebase (HCHB) or similar EMR
• Wound care exposure
