Licensed Vocational Nurse (LVN)

Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care through technology and skilled clinical support.

Responsibilities

• Deliver skilled nursing visits in the home within LVN scope under RN/clinical supervision
• Perform focused assessments and monitor for changes in condition
• Escalate clinical changes promptly to the RN/MD/clinical team
• Support medication reconciliation and medication safety education
• Provide patient and caregiver education using teach-back methods
• Support wound care and other skilled tasks per orders
• Coordinate with the interdisciplinary team to ensure visit adherence
• Document thoroughly and timely in Homecare Homebase (HCHB)

Requirements

• Current unrestricted Texas LVN license
• CPR (BLS) certification
• Current Texas driver’s license
• Comfort with daily travel across assigned territory
• Strong communication and documentation skills

Preferred Qualifications

• Prior home health or hospice experience
• Familiarity with Homecare Homebase (HCHB) or similar EMR
• Wound care exposure
