Licensed Vocational Nurse

Responsibilities

• Provide direct nursing care and treatments to PACE participants
• Administer medications and injections, monitor vital signs, and observe health status
• Conduct triage of participants presenting with acute or urgent needs
• Perform assessments and document participant conditions and responses to treatments
• Assist with intake evaluations, wound care, and chronic disease management
• Implement care plan interventions and support participant caseload management
• Coordinate care transitions between home, day center, and other settings
• Maintain accurate and timely documentation in electronic health records (EHR)
• Collaborate with interdisciplinary teams including physicians, therapists, and social workers
• Build therapeutic relationships and educate participants and caregivers on health topics

Requirements

• Current California LVN license in good standing
• Minimum of 1 year nursing experience, preferably in geriatrics or home health
• CPR/BLS certification
• Experience with electronic medical records (EMR)
• Valid driver's license and reliable transportation
• Ability to work in clinic, day center, and participant homes

Preferred Qualifications

• Previous PACE program experience
• Geriatric certification or specialized training
• Experience with interdisciplinary team models
• Bilingual skills (e.g., Spanish, Cantonese, Tagalog)
• Familiarity with value-based care models
