About the Company
Adaptive Home Health is building a higher-acuity, patient-centered, skilled home health model across Texas. Our mission is to dramatically improve patient access to home health care through technology and clinician-focused operations.
Responsibilities
- Deliver skilled nursing visits in the home within LVN scope under RN/clinical supervision
- Perform focused assessments and monitor for changes in condition
- Escalate clinical changes promptly to the RN/MD/clinical team
- Support medication reconciliation and medication safety education
- Provide patient and caregiver education using teach-back methods
- Support wound care and other skilled tasks per orders
- Coordinate care with the interdisciplinary team to ensure visit adherence
- Document thoroughly and on time in Homecare Homebase (HCHB)
Requirements
- Current unrestricted Texas LVN license
- Comfort working in a field-based workflow
- Strong communication and documentation skills
- Current Texas driver’s license and comfort with daily travel
Preferred Qualifications
- Prior home health or hospice experience
- Familiarity with Homecare Homebase (HCHB) or similar EMR
- Wound care exposure
