Licensed Practical Nurse/RN - Respite

Responsibilities

• Provide education and consultation to clients and staff on medical health, medications, and general medical topics
• Deliver one-on-one training tailored to specific individual needs
• Develop and lead groups and trainings for clients and staff on general health and wellness topics
• Assist clients in accessing community resources that support their health and wellness goals
• Meet with clients and staff to discuss medical diagnoses, causes, symptoms, and treatment options
• Perform direct medical care including monitoring vitals and wound care as ordered by physician
• Review medical documentation and provide consultation and coordination of care
• Coordinate overall medical care for clients and ensure necessary follow-up is completed
• Support the team by introducing tools for symptom assessment and evaluating treatment effectiveness

Requirements

• LPN or RN license
• Experience working with adults with severe mental illness
• Valid Massachusetts drivers license
