Licensed Practical Nurse

Responsibilities

• Conduct Home Health visits in the Needham, Newton, and Wellesley areas on a weekday schedule (Monday - Friday, 8:00 AM - 4:30 PM).
• Provide care and assistance to patients with various medical needs, including wound care, wound vacs, trach and suctioning, catheters, ostomies, phlebotomy, CHF, COPD, and diabetes.
• Collaborate with the healthcare team to ensure high-quality patient care.
• Participate in an extensive Home Health orientation program, including 6-8 weeks of field preceptorship and support from an Educator.
• Utilize the Home Care Home Base EMR system for patient documentation.

Requirements

• Completion of an LPN Program.
• Valid LPN license in the state of Massachusetts.
• Ability to travel to patient homes in the Needham, Newton, and Wellesley areas.
