Licensed Practical Nurse

Responsibilities

• Provide high-quality clinical services within the scope of practice as defined by state laws and the plan of care
• Deliver direct nursing care to acutely and/or chronically ill patients in their homes
• Document patient visits thoroughly and completely in the electronic medical record per regulatory requirements
• Coordinate patient care activities and report changes in condition to the Registered Nurse and healthcare team
• Participate in the agency's care coordination process, including attending patient case conferences

Requirements

• Current and unrestricted Licensed Practical Nurse (LPN) licensure in the state of practice
• Current CPR certification or the ability to complete certification within 90 days of hire
• Current driver's license and vehicle insurance
• Access to a dependable vehicle or ability to use public transportation

• Strong ability to manage multiple tasks simultaneously
• Proven ability to work independently
• Excellent communication, writing, and organizational skills
