Licensed Practical Nurse

Responsibilities

• Perform initial and ongoing nursing assessments of assigned residents to identify health needs and monitor changes in status
• Develop, implement, and evaluate individualized plans of care in collaboration with the interdisciplinary team
• Provide direction and supervision to Health Care Aides to ensure safe and competent resident care
• Document accurate and timely health information and care plans in accordance with regulatory requirements and company policies
• Assist residents with physical requirements and activities of daily living
• Monitor unstable or ill residents and coordinate with Registered Nurses, physicians, or case managers as necessary
• Participate in quality improvement activities, staff development, and the orientation of new employees or students

Requirements

• Graduate of an approved Licensed Practical Nurse (LPN) program
• Active CLPNA registration
• Current First Aid & CPR certification
• Non-Violent Crisis Intervention certification
• Class 5 driver’s license with a clean driver’s abstract
• Successful completion of a Criminal Police Investigation Check

Preferred Qualifications

• Two years of experience in a continuing care or mental health setting
• Proficiency with MS Word and MS Excel
