Licensed Practical Nurse

Responsibilities

• Perform initial and ongoing nursing assessments for assigned residents to identify health needs and monitor changes in status.
• Develop, implement, and evaluate individualized plans of care in collaboration with the interdisciplinary team.
• Provide support and direction to Health Care Aides, serving as a team leader when required.
• Document accurate and timely health information, including data collection and care interventions, using electronic charting systems.
• Administer nursing treatments and interventions using evidence-based knowledge.
• Coordinate with Case Managers, Social Workers, and physicians regarding changes in resident status or discharge planning.
• Participate in quality improvement activities, staff development, and the orientation of new employees or students.

Requirements

• Graduate of an approved Licensed Practical Nurse (LPN) program.
• Active CLPNA registration.
• Current Basic Life Support (BLS) certification.
• Non-Violent Crisis Intervention certification.
• Valid Criminal Police Investigation Check.
• Class 5 driver’s license with a clean driver’s abstract.

Preferred Qualifications

• Two years of experience in a continuing care or mental health setting.
• Proficiency with MS Word, MS Excel, and email.
