Licensed Practical Nurse

Responsibilities

• Provide high quality clinical services within the scope of practice as defined by state laws
• Perform technical skilled care in the patient's home
• Document patient visits thoroughly in the electronic medical record
• Participate in care coordination and report changes in patient condition
• Communicate effectively with patients, families, physicians, and agency management

Requirements

• Current and unrestricted LPN licensure in the state of practice
• Current driver's license and vehicle insurance
• Access to a dependable vehicle or public transportation

Preferred Qualifications

• Current CPR certification or ability to complete within 90 days of hire
• Previous home care experience
• Ability to manage multiple tasks simultaneously
• Ability to work independently and flexible hours
• Strong communication, writing, and organizational skills
