Licensed Practical Nurse

Responsibilities

• Provide high-quality clinical services within the scope of practice and in accordance with the patient plan of care
• Perform technical skilled care in the patient's home following all applicable infection control procedures
• Document patient visits thoroughly and completely in the electronic medical record per regulatory requirements
• Actively participate in the care coordination process, including reporting changes in patient condition
• Communicate effectively with patients, families, physicians, and other members of the healthcare team

Requirements

• Current and unrestricted LPN licensure in the state of practice
• Current CPR certification
• Current driver's license and vehicle insurance
• Access to a dependable vehicle or ability to use public transportation
• Ability to work flexible hours to meet identified client needs
• Ability to function in any home situation regardless of client demographics or financial condition

Preferred Qualifications

• Previous experience in home care settings
• Ability to manage multiple tasks simultaneously
• Proven ability to work independently
• Strong communication, writing, and organizational skills
