Licensed Practical Nurse

Responsibilities

• Provide direct patient care as defined in the State of Nursing Practice Act
• Implement plan of care indicated by the clinician
• Provide accurate and timely documentation consistent with the plan of care
• Assess and provide patient and family/caregiver education and information
• Participate in coordination of home care services and report needs to case managers
• Use equipment and supplies effectively and efficiently
• Participate in personal and professional growth and development

Requirements

• Graduate of an accredited practical nurse or vocational nursing program
• Current state license as an LPN/LVN
• 1+ years experience as a nurse in a clinical care setting
• Ability to work collaboratively as part of an interdisciplinary team
• Excellent verbal and written communication skills
• Strong organizational and time management abilities
• Solid clinical judgement
• Reliable transportation and valid automobile insurance coverage

Preferred Qualifications

• Experience in home health or hospice
