Licensed Practical Nurse

Responsibilities

• Provide direct patient care as defined in the State Nurse Practice Act, including specialized end-of-life care
• Implement nursing practice following comprehensive assessment and the plan of care
• Provide accurate and timely documentation of patient services
• Assess and provide patient and family/caregiver education regarding diagnosis and self-plan of care
• Coordinate hospice services and report identified needs to the interdisciplinary group
• Use and prepare equipment and supplies effectively using aseptic technique
• Provide appropriate pain and symptom management and evaluate patient response
• Participate in professional growth and quality assessment performance improvement activities

Requirements

• Graduate of an accredited practical nurse or vocational nursing program
• Three years of nursing experience
• Current LPN/LVN license in the State
• Current CPR certification
• Licensed driver with a reliable, insured automobile
• Understanding of hospice philosophy and the needs of the terminally ill

Preferred Qualifications

• Community health or hospice experience
• Medical/surgical experience
