Licensed Practical Nurse (LPN)/Licensed Vocational Nurse (LVN)

Responsibilities

• Serve as the primary on-site point of contact for unannounced Medicare/MAC site inspections.
• Escort inspectors through the facility and respond to operational questions professionally.
• Ensure facility readiness by verifying utilities, equipment, and signage are operational each morning.
• Maintain the Readiness Binder and ensure strict physical HIPAA compliance for medical records and server areas.
• Facilitate patient enrollment into the Chronic Care Management (CCM) portal.
• Conduct daily outbound calls and text messages to patients to document health status and maintain engagement.
• Collect and document patient medical histories, current health status, and evolving symptoms.
• Distribute evidence-based health support resources, such as stretching guides and exercise videos, to patients.

Requirements

• Active LPN or LVN license in good standing in the state of Arizona.
• Minimum 3 years of experience as an LPN or LVN in a clinical or remote care setting.
• Proficiency with Zoom, Slack, and online patient enrollment platforms.
• Strong organizational skills and the ability to work independently.
• Demonstrated commitment to HIPAA compliance and patient privacy.
• Ability to maintain a consistent physical presence at the Phoenix facility during mandatory windows.

Preferred Qualifications

• Background in healthcare administration, clinic operations, or front-desk management.
• Clinical experience in Orthopedics, Physical and Rehabilitation Medicine, Sports Medicine, or Neurology.
