Licensed Practical Nurse (LPN)

Responsibilities

• Provide high-quality home health care to patients within the Leominster Team Area
• Follow weekday schedules (Monday - Friday, 8:00 AM - 4:30 PM)
• Deliver patient care in various home settings across assigned territories including Ashburnham, Ashby, Fitchburg, Gardner, Leominster, Westminster, and Sterling
• Maintain accurate patient documentation and clinical records

Requirements

• Completion of an LPN Program
• Current license to practice in the state of Massachusetts

Preferred Qualifications

• Previous experience in Home Health care
• Experience with wound care and wound vacs
• Experience with trach and suctioning, catheters, or ostomies
• Experience with phlebotomy
• Knowledge of managing CHF, COPD, and diabetes
