Licensed Practical Nurse (LPN)

Responsibilities

• Provide high-quality home health care to patients in the assigned territory
• Follow weekday schedules (Monday - Friday, 8:00 AM - 4:30 PM)
• Deliver patient care including wound care, catheter management, and medication administration
• Maintain accurate patient documentation and clinical records

Requirements

• Completion of an LPN Program
• Current license to practice in Massachusetts
• Ability to travel within the assigned territory (Grafton, Northbridge, Mendon, Milford)

Preferred Qualifications

• Previous experience in Home Health care
• Experience with wound care and wound vacs
• Experience with trach and suctioning, catheters, or ostomies
• Experience with phlebotomy
• Knowledge of CHF, COPD, and diabetes management
