Licensed Practical Nurse (LPN)

Responsibilities

• Interact with patients, observe behaviors, and communicate significant observations to the multidisciplinary team
• Administer medications and provide patient education
• Utilize de-escalation skills to maintain safety in volatile situations
• Coordinate admission, orientation, education, and discharge planning
• Consult with the treatment team on master treatment and discharge plans
• Obtain vital signs, monitor symptoms, and document in the patient record
• Maintain a clean, orderly, and safe environment
• Oversee quality assurance and compliance activities
• Provide medical updates to utilization review and managed care representatives

Requirements

• Current, unencumbered, active LPN license in the state of practice
• Minimum 21 years of age
• Current nationally accredited CPR/First Aid/AED certification
• Knowledge of computer and electronic medical records (EMR) systems
• Ability to work with a diverse patient population and maintain professional boundaries
• Understanding of ethical standards and scope of licensure

• Excellent organizational and communication skills
• Proficiency with Microsoft Office Suite
• Ability to multi-task and prioritize in a fast-paced environment
• Strong interpersonal skills and attention to detail
