Licensed Practical Nurse (LPN)

Responsibilities

• Provide hands-on nursing care for older adult patients in a long-term care environment
• Take vital signs, document care, and maintain clear patient records
• Administer prescribed medications, monitor reactions, and assist providers during exams and treatments
• Support patients with activities of daily living, including dressing, bathing, eating, and comfort measures
• Provide end-of-life care as appropriate
• Educate families and participate in care planning with the medical team
• Follow safety policies, care procedures, and assigned nursing workflows

Requirements

• High school diploma or GED
• Graduate of an accredited LPN program
• Active and unrestricted Ohio LPN license
• Current BLS from the American Heart Association or Red Cross
• 6 months of LPN experience
