Licensed Practical Nurse (LPN) - Home Health

Responsibilities

• Provide direct patient care through home visits within the scope of practice assigned by a Registered Nurse
• Observe patient progress and communicate findings to the RN to assist in adjusting plans of care
• Document all nursing interventions, medications, and treatments according to agency policy
• Administer medications and treatments as prescribed by the patient's physician
• Teach patients and their families preventative and rehabilitative measures to increase independence
• Assess the learning needs of patients and caregivers to provide appropriate instruction
• Maintain ongoing communication with the interdepartmental team regarding patient needs and changes

Requirements

• Current Illinois practical nursing license
• Current BLS certification through the American Heart Association (AHA)
• 1-2 years of recent acute care experience in an institutional setting
• Valid driver's license and current automobile insurance
• Access to a reliable and dependable mode of transportation for field visits
• Strong observation, verbal, and written communication skills
