Licensed Practical Nurse (LPN) - Home Health

Responsibilities

• Provide skilled nursing care and case management in a home health setting
• Collaborate with an interdisciplinary team to develop and implement individualized care plans
• Perform patient and family education to promote independence and optimal clinical outcomes
• Maintain accurate and timely clinical documentation

Requirements

• Current LPN license in the State of Indiana
• Graduate of an accredited school of nursing
• One year clinical experience in nursing
• Valid driver’s license and dependable transportation
• Strong clinical assessment, communication, and organizational skills

Preferred Qualifications

• Home health experience
• Critical Care nursing experience
