Licensed Practical Nurse (LPN) - Home Care

Responsibilities

• Deliver age-appropriate nursing care to patients in their homes following the plan of care established by a Registered Nurse
• Document accurate and ongoing treatments, including nursing interventions, patient responses, and progress toward goals
• Engage patients and caregivers in meaningful education regarding their care and limitations
• Communicate patient progress or changes in status to the interdisciplinary team and ordering providers
• Participate in agency projects, committees, and quality management programs as requested

Requirements

• Graduate of an accredited school of Nursing
• Current New Hampshire nursing license or a compact state license (NLC)
• One year of recent medical/surgical or skilled/acute care nursing experience
